What Octagos Does
Octagos provides a cloud-based, vendor-agnostic software and services platform for in-clinic and remote cardiac monitoring of implantable cardiac devices (CIEDs). Every incoming transmission is processed through the Two Brain Approach: the “First Brain” (Atlas AI) performs preliminary analysis, and the “Second Brain” (an IBHRE-certified U.S.-based device specialist) reviews and finalizes. Physician e-signature is required to complete every report. The platform adds bi-directional EHR and manufacturer-portal integration, AutoBiller automated documentation and billing, and a dedicated patient engagement team.
Octagos maintains a strong security posture, provides 24/7 production support, and is known for excellent analytics and research-grade data for teams interested in clinical research. It is also a strong option for groups converting from Paceart, with a structured, no-cost historical data migration.
Service Models
All tiers include EHR integration, training, licenses, mobile apps, software upgrades, and 24/7 production support at no additional cost. Tier transitions are available with 30 days written notice.
| Tier | What's Included |
|---|---|
| Software-Only | Platform access, EHR integration, onboarding, training, and support. |
| Software + Patient Engagement | Everything in Software-Only, plus full-service patient calls and scheduling by the Octagos engagement team. |
| Full-Service | Everything in Software + Patient Engagement, plus IBHRE-certified U.S.-based device specialist review of all remote transmissions. |
Full-Service pricing options: Per Patient per Month, Per Billable Transmission, or IDTF (Independent Diagnostic Testing Facility) model.
Bi-Directional Integration
Octagos is known for robust, bi-directional integrations performed at no cost to the customer. Data flows in both directions — Octagos both pulls from and pushes to EHRs and cardiac device manufacturer portals. Integration is built and maintained entirely in-house using Mirth as the interface engine, with no third-party middleware and no additional EHR implementation costs. Octagos is a strong option for groups converting from Paceart, offering a structured, no-cost, one-time historical data migration.

The Two Brain Approach & Alert Classification
Alerts are configurable per clinic, device type, and manufacturer. Octagos responds to urgent alerts within 1–2 hours, exceeding HRS guidance of one business day.
Alert Classification
| Severity | Indicator Color |
|---|---|
| High | Red |
| Medium | Yellow |
| Low | White |
AutoBiller
AutoBiller automatically captures billable events on transmission receipt, applies diagnosis and CPT codes, and drops charges into the EHR once reports are e-signed — no manual intervention for standard billable encounters. If a patient misses a scheduled download, AutoBiller generates a billable event from any transmission received during the monitoring period, keeping billing cycles intact.

Atlas AI: Peer-Reviewed Evidence
Atlas AI is the only cardiac-monitoring AI peer-review validated head-to-head against human device technicians. In “Role of Artificial Intelligence in Reducing Data Deluge from Cardiac Implantable Electronic Devices” (JACC: Advances, Vol. 4, No. 10, October 2025; DOI 10.1016/j.jacadv.2025.102057), researchers evaluated 690,673 transmissions from 74,217 devices across 78 U.S. clinics.
Head-to-Head Results (JACC: Advances, 2025)
| Approach | Sensitivity | Accuracy | Transmissions Forwarded |
|---|---|---|---|
| High | 99.1% | 93.8% | 22.8% |
| Medium | 70.8% | 95.4% | 38.5% |
| Low | 99.1% | 99.5% | — |
The Two Brain Approach achieved 99.5% accuracy, 99.1% sensitivity, and 99.8% specificity. A companion finding across 32,721 patients showed 57% of transmissions were redundant or non-actionable, with 60%+ reduction in physician workflow burden and 200+ staff hours saved per 1,000 patients per month.
